1. Self-health form
2. Visa issuance form
+2 copies
3. 4 passport sized
photos
4. Signed employment
contract + 3 copies
5. 2 copies of new
passport signature page
6. 2 copies of resume
signed
7. Original Federal
apostilled FBI criminal background check + 2 copies

8. Original apostilled
and notarized copy of diploma + 2 copies
